And this evening, I talked to the sculptor who owns that house. He told me that he scavenged the marker from a back yard elsewhere in West Somerville (also nowhere near the city line), where it was being used to prop something else up.
He liked the way it looked, so he erected it where you see it now. He doesn't have any more clue than I do where it actually belongs. For all we know, it could belong to Concord and Sudbury, or Canton and Sharon, or some other pair of adjoining C and S towns we haven't thought of yet.
