One-month permit for rental cars?

Has anybody gotten this permit before? It's on the city website but it doesn't say much about it. Specifically, I'd like to know if this is something that has to be registered to the car, or if this is a "floating" permit like the visitors passes are; i.e. do you have to pick up the rental car before you can apply for the permit?

This is correct as of November 2009, when last I got one of these permits.
You need the rental agreement, the plate number, and proof of Somerville residency.
