Would the scum at the bottom of a bottle of Bragg's apple cider vinegar do the trick? They claim to have a live mother, but they also claim other things on their bottles that I would take with many grains of salt. :-P
(Specifically, I doubt that the USDA or FDA regulates this claim, so it may not actually be live.)
"Raw, unfiltered, no preservatives". Darned if I know how to get the stuff out, though -- the bottle has a narrow neck (so I can't use the turkey baster), and even a small amount of handling stirs up the stuff to a surprising degree (so pouring off the majority of the liquid into a different bottle might not work.)
1) We go through it pretty slowly. You can get this brand at Cambridge Naturals, though!
2) I guess I could try filtering the dregs through a coffee filter.
3) I just found another bottle with a tiny amount of liquid in it but definitely some sediment. It's probably a year old. I have *no idea* how hardy mother-of-vinegar is...
